Flow Health and Wellness, LLC is a private, home‑based boutique practice offering a blend of wellness services and licensed occupational/physical therapy services. The practice operates by appointment only, and the exact address is provided after booking to protect client and practitioner privacy.
By scheduling an appointment, purchasing a package, or receiving services, clients agree to the terms outlined below.
General Information
Scope of Services
Flow Health & Wellness provides two distinct categories of services:

Clinical OT/PT Services — Provided only when medically appropriate and within the licensed scope of practice. These services may require an evaluation, documentation, and adherence to state practice laws.

Wellness Services — Non‑clinical, relaxation‑focused, and not intended to diagnose, treat, or manage medical conditions. Examples include wellness lymphatic drainage massage, mobility sessions, and general wellness education.
Clients will be informed of the category of service they are receiving. Wellness services are not a substitute for medical care.
Booking & Appointments
- All sessions are by appointment only.
- Clients are responsible for booking, rescheduling, and managing their appointments.
- The practice address is shared only after booking to maintain privacy and safety.
Cancellation Policy
- A minimum of 24 hours’ notice is required to cancel or reschedule.

Late cancellations or no‑shows will result in the full session fee or deduction from a package.

Repeated missed appointments may result in refusal of future bookings.
Refund Policy
- All sales are final.
- Refunds are not provided for client cancellations, missed appointments, or scheduling conflicts.
- Refunds or credits are only issued if the therapist cancels and an appointment cannot be rescheduled.
Packages & Expiration
- Packages are non‑refundable, non‑transferable, and non‑shareable.

All packages expire 6 months from the purchase date unless otherwise stated.

Unused sessions cannot be extended or credited.
Client Responsibilities
Clients agree to:

Provide accurate health information during intake.

Update the practitioner on any changes in health status.

Communicate preferences, discomfort, or concerns during sessions.

Follow all studio policies and respect the home‑based environment.
Flow Health & Wellness reserves the right to discontinue services if a client’s needs fall outside the scope of practice or wellness care.
Practitioner Rights
Flow Health and Wellness may:

Decline or discontinue services when appropriate.

Reschedule appointments due to illness, emergencies, or unforeseen circumstances.

Client information is kept confidential in accordance with state and federal regulations.

The practice location is private and shared only with scheduled clients.

Photography, recording, or sharing of the practice environment without permission is prohibited.
Privacy & Confidentiality

Clients understand that:

Flow Health & Wellness, LLC is not liable for outcomes related to undisclosed health conditions or failure to follow recommendations.
